The last few days, at the Canadian Medical Association's ("CMA") annual convention, Canada's doctors have fought a nasty little hand-to-hander over whether Canada's health care system should be publicly owned or privately owned. In fact every doctor's office is a private little business although doctors have the well-earned reputation of being the universe's worst business people. Obviously, the concern, at root, is not for the patient but for the doctor (it looks as though Dr. Brian Day of Vancouver will win the contested presidency he owns the largest private clinic in Canada -- detractors call him "Dr. Profit"). Why don't the doctors spend their valuable time cleaning up the issues that matter to patients:

  • Canada lacks relational data base systems which keep track of patients, their histories, the treatments and drugs they're taking and how drugs interact with other drugs. Prescription drug addicts have a hay day; Doctors prescribe drugs which interact, often, we imagine, fatally, with other drugs. We're talking about a data base simpler than Wal-Mart's.
  • There is a shortage of doctors in Canada but qualified doctors immigrating from other countries can't get through the red tape thrown up by the CMA and provincial medical associations - some immigrant doctors are working, no doubt, in Wal-Mart stores and hamburger joints and at cleaning offices.
  • Canadian taxpayers are spending we estimate $1,000,000 per graduated Canadian doctor yet many doctors make a beeline for work over the U.S. border where private hospitals pay big salaries - no one is standing at the border to collect the cost of the doc's education.
  • The doctors have allowed themselves to bullied about by hospital administrators who are paid more than the docs but whose only competencies seem to be building bureaucratic empires and bullying doctors.
  • Not every qualified doctor has "hospital privileges" - hospital privileges mean more patients and more fees - doctors play keepers at the gate of each hospital, excluding many of their colleagues from hospital privileges - J. P. Morgan advised aspiring entrepreneurs to "take a conflict of interest and exploit it" - exactly what the docs on the hospital privileges boards have done.
